## Investigation State

  1. Defect 1: MLLM Multimodal Facade confirmed. Media was never bound to contents/inputs. Remediation extracts OpenCV JPEG keyframe bytes (for shots) and pairwise junction frames (for scenes) and attaches them via `genai.types.Part.from_bytes` and `agy.from_bytes`. If offline/no API key, cleanly returns None to delegate to `OfflineHeuristicEngine`.
  2. Defect 2: Scene Gate missing/corrupt files false-positive approval (0.95 APPROVE) confirmed. Silent `continue` loops and empty defaults caused approval. Remediation implements upfront validation of every file (exists, >0 bytes, cv2.isOpened, frames > 0) returning 0.0 RETAKE_SHOT immediately upon failure.
  3. Defect 3: Frozen/defective video approval leak (0.85 APPROVE) confirmed. Single visual defect only docked 0.15. Remediation docks >= 0.35 for severe defects (frozen, black frame, duration < 1s, low resolution), forces score <= 0.65, approved=False, and suggested_action="RETAKE_SHOT".
  4. Defect 4: Dead code in `_find_character_portrait` and bypass of character check for non-Kim Trọng characters confirmed. Thúy Kiều video evaluated as Vương Ông received 1.0 APPROVE. Remediation wires `_find_character_portrait` for all characters and cross-correlates against Thúy Kiều (>0.85 vs <0.60) to flag cross-contamination, docking 0.50 and forcing RETAKE_SHOT.
  5. Defect 5: Audio stream inspection in `_check_audio_stream` confirmed ignoring stdout. Remediation parses `json.loads(res.stdout)` to verify audio stream presence, AAC/PCM/MP3 codec, and valid channel count (>0).
